Swim Team Application Clear Fork Swim Team Application – 2025. Please fill it out in it’s entirety. A swimmer must compete in 2 meets (in the GMAC) for swimming and 2 for diving to participate in the championship meet. (The Colonel Crawford invitational and the Clear Fork invitational do not count.) All swimmers will need to be a member of the Clear Fork pool and have all forms and money turned in before entering the water for practice or a meet. SWIM AND DIVE TEAM FEES– Cost is $67.00 per swimmer and $52.00 for each additional swimmer. – Payment must be received after completing the application to be considered completed.– Registration includes a $15 concessions fee in lieu of donating specific items for the concession stand at swim meets.
